We want to make your first visit with us as stress-free as possible. To make this happen, we ask that you complete the following items.
Bring with you:
- All printed and completed forms in our New Patient Packet, including:
- Patient Information
- Eaglesoft Medical History
- Patient Consent
- Acknowledgment of Receipt of Notice of Privacy Policies
- Your current Dental Insurance Card
- An updated copy of all of your Current Medications
Records Transfer
If you wish to have your dental records sent from your previous dental office, you will need to print and complete our Records Release form and send it to your previous provider.
